Last week I promised you the full breakdown on pet insurance in Portugal, and today I’m delivering both the full guide and my nutshell version of what I found.

And after all that research, what did I discover for our household?

Well, firstly, our dog is 12, so unfortunately she’s too old for most of the health insurance options. No insurance there.

Then there are our four adopted cats, some of whom are slightly feral and all of whom have their own little backstories. They missed all their first shots, and one already has pre-existing issues, which are generally excluded from cover. So the insurance policy that would actually matter in his case might just eat away at our retirement fund, while not necessarily covering the thing we'd be most worried about anyway.

Are we considering pet insurance then? Nope.

I think I’ll put the cats on OnlyFans and they can start saving towards any medical bills that may crop up. 😂

Which brings me to something else I’ve been thinking about. If you do have pets, keeping their paperwork, vaccinations and other documents up to date is one of those boring little jobs that becomes very important when you actually need it.

🐕‍🦺 Pet Insurance in Portugal: The Nutshell Version

What the 2026 comparison data actually shows.

Portugal has a reputation for affordable vet care, and the numbers mostly support it. A routine consultation averages around €28. An emergency room visit can still clear €500, and orthopaedic surgery is reported at around €1,200.

So the question is not whether Portuguese vet care is expensive. It is what happens the one time it is.

I spent a week on this: the DECO PROteste 2026 test of 30 policies across 10 insurers, the policy documents those insurers publish, Portuguese legislation, and complaints filed against named insurers on Portugal's two main consumer platforms.

One caveat before anything else. DECO's comparison covers ten insurers, not every insurer in Portugal. So everything below describes that sample rather than the whole market, and your own condições particulares override any general guide, including mine. I am not a broker and this is not advice.

Three things that circulate widely in English-language guides did not match what I found.

The three corrections

1. Most of these policies are not pay-and-claim. This is the big one. DECO reports that the majority of the policies it tested work within contracted clinic networks. Among the ten insurers tested, only Ocidental and Barkibu have no contracted network. Fidelidade offers both models. DECO gives around €12.50 as a typical network price for a consultation that would otherwise cost around €30.

2. Waiting periods are longer than usually reported. They vary by insurer and by cover. Fidelidade publishes 60 days for consultations, 90 for surgery and 180 for funeral cover, with no waiting period for accident-related treatment. Barkibu policyholders have reported waits of around six months for certain treatments.

3. Premiums in the 2026 sample run €87 to €485 a year, a wider spread than the €100 to €200 often quoted. The highest-rated policy in the test, Fidelidade Pet Vital, comes out at €264 to €408 depending on the animal.

The DO list

DO think about age early. In the policies tested, entry ages are earlier than many English-language guides suggest. Ocidental and Seguros Continente close health cover at 7, Fidelidade at 8, and Barkibu at 11, the widest in the sample. Some insurers offer other forms of cover to older animals.

DO check the age-3 condition. Mapfre, Seguros Continente and N Seguros require enrolment before age three for lifetime surgery cover. Petki uses age four. After that, surgery cover ends at the tenth birthday. If you adopt a five-year-old rescue, which many of us do, that window has already closed at those insurers.

DO pull the network list for your postcode. Not the region, the postcode. On the Silver Coast this weighs more heavily than in Lisbon. Many insurers do allow out-of-network reimbursement where there is no contracted provider near your home, which is worth confirming in writing before you sign.

DO ask two questions in writing. How do you treat presumed-hereditary breed conditions? Is the deductible applied per claim, per consultation, or per diagnosis? Both have been the subject of documented complaints.

DO use the free-look period. Under the Fidelidade conditions I reviewed, buying at a distance gives you 14 days from receiving the policy to cancel without reason or penalty. Useful time in which to read where the deductibles and waiting periods actually sit.

DO give your NIF at the vet counter. Veterinary expenses count towards the IRS VAT-invoice deduction: 15% of the VAT on services and 35% on medicines, within the household cap. Rules change, so confirm the current position for your tax year.

The DON'T list

DON'T assume a plan is insurance. Medicare's Vetecare is presented by its provider as access to pre-negotiated rates at partner clinics, not as an ASF-regulated insurance contract. No underwriting and no waiting period, but also no capital and no claim to make. An out-of-network emergency is not something a discount plan is structured to pay for.

DON'T assume cheap is a saving. The four policies priced under €96 for a five-year-old mixed-breed dog score 41 to 44 out of 100 on DECO's quality index. Worth weighing against the premium saved.

DON'T stop at the headline capital. Petki Platinum advertises €20,000 a year for surgery, but DECO notes caps of €3,000 per surgery type and €1,000 per claim, which in practice limits most single events to €1,000. Ask for the per-claim limit every time.

DON'T assume good cover means fast claims. Fidelidade Pet Vital carries the highest coverage-quality score in DECO's test. Separately, the public complaint record shows recurring themes around reimbursement delays and disputed pre-existing-condition refusals. Those are different questions, and a quality index answers only the first. Complaint records are self-selecting, so they show what kinds of disputes arise rather than how often.

DON'T overlook the clauses aimed at people like us. Fidelidade's conditions state that cover ceases once the animal has been abroad for more than 90 days, and if the policyholder stops being habitually resident in Portugal. If you spend months at a time back home with the dog, check the equivalent clause in your own policy.

If your dog might be on the list

Portuguese law does not generally require health insurance for pets, but it does require civil liability insurance for certain dogs: Cão de Fila Brasileiro, Dogue Argentino, Pit Bull Terrier, Rottweiler, American Staffordshire Terrier, Staffordshire Bull Terrier and Tosa Inu, plus crossbreeds. The framework is Decreto-Lei 315/2009.

Current insurer documentation states the legal minimum capital as €50,000 per year per animal. That figure is set by ministerial order and can be revised, so confirm it when you buy. Fidelidade's standard Pets product states it is not intended for animals classified as dangerous, so a standalone liability policy may be needed. Mapfre applies a 10% deductible with a €125 minimum on these claims.

Breed identification is not always legally straightforward. If there is doubt, that is worth resolving with a vet or your junta rather than assuming.

The honest summary

Across a healthy young animal's first six years, many owners will pay in more than they get out. That is how insurance works.

What points the other way is the tail risk, and the fact that the window narrows. On the entry ages in this sample, options reduce sharply after 7 or 8, and lifetime surgery guarantees may close at 3 or 4. Self-insuring is a decision that is easier to make than to reverse.

If you would rather self-insure, that is a reasonable position. But €30 a month actually transferred to a separate account is €2,160 by year six. €30 a month intended and not transferred is nothing.
